USMLE Step 1 Tutoring Frequently Asked Questions

The USMLE Step 1 assesses the core basic sciences that underpin clinical care — anatomy, physiology, biochemistry, microbiology, pathology, pharmacology, molecular and cell biology, and behavioral sciences—along with cross-disciplinary areas such as genetics, immunology, and ethics. The emphasis is on applying these concepts to clinical scenarios, not on rote memorization.

With 280 questions testing integrated knowledge across all body systems, many students find that working with a Step 1 tutor helps them master the complex interconnections between basic science concepts and clinical applications.

Step 1 is administered as a computer-based exam at Prometric testing centers worldwide year-round. The exam consists of seven 60-minute blocks with up to 40 questions each, totaling 280 questions over 8 hours. You'll have a minimum of 45 minutes of break time, plus an optional 15-minute tutorial that can be skipped for extra break time.

Testing is available throughout the year except major holidays, with peak periods in May-July and November-December when scheduling fills up quickly.

You must select a 3-month eligibility window and can schedule up to 6 months in advance. Many students preparing with USMLE Step 1 tutoring find it helpful to practice with the exact computer-based format and timing constraints before test day.

Since January 2022, Step 1 uses a pass/fail scoring system only — no numerical scores are reported. You need to answer approximately 60% of questions correctly to pass, which corresponds to the internal passing threshold of 196 on the former 3-digit scale.

Results are typically available 2-4 weeks after testing, though you should allow up to 8 weeks.

Preparation time varies based on your foundation and time since completing basic sciences. For current medical students, 6-8 weeks of dedicated study is recommended, though 3-4 months total preparation is optimal. Students who've been away from basic sciences longer typically need 4-6 months of preparation.

Our tutoring service generally recommends 2-6 months for medical students, with 6-8 hours of daily study combining content review and practice questions. The key is taking the exam as soon as possible after completing basic sciences while concepts are fresh.

Many students find that structured USMLE Step 1 tutoring helps them create an efficient study timeline and avoid common preparation pitfalls that can extend study periods unnecessarily.

The number of tutoring hours depends on your baseline knowledge, study timeline, and specific needs. Most students benefit from 20-40 hours of USMLE Step 1 tutoring spread over their preparation period.

Students with strong foundations may need 15-25 hours focused on high-yield topics and test-taking strategies, while those requiring more comprehensive review often benefit from 30-50 hours.

Sessions are typically scheduled 2-3 times per week during your dedicated study period. A USMLE Step 1 tutor can help assess your needs during an initial consultation and recommend a personalized plan. Many students start with a smaller package and add hours as needed, allowing for flexible adjustment based on practice exam performance and confidence levels.

If you fail Step 1, you'll receive performance feedback by content area to guide your preparation for retaking the exam. Given the high stakes of the pass/fail system, many students invest in USMLE Step 1 tutor support to ensure they pass on their first attempt.

You can retake it up to 4 times total, with a maximum of 3 attempts in any 12-month period. You must reapply and pay full fees ($670) for each retake, and there's no mandatory waiting period between attempts 2-3. However, a fourth attempt requires at least 12 months from your first attempt and 6 months from your most recent attempt.

Failed attempts permanently appear on your USMLE transcript, which can impact residency applications and may limit specialty choices. About 71% of students pass on retake attempts. Most successful retakers study for 3-6 months and use the performance feedback to target weak areas.
